The usual causes are radiators or piping that have lost their pitch, a wet return, insufficient main venting, or a boiler running at higher pressure than it needs. On a hot-water system, knocking is more often air in the loop or an oversized circulator. In nearly every case this is a repair, not a reason to replace the boiler — despite what you may have been told.
On steam, cold radiators usually point to failed air vents, a pressure setting that is too high, or valves that are partially closed. On hot water, it is typically a failed circulator, a stuck zone valve, or air trapped in that zone. It is rarely the boiler itself, which is why diagnosing the whole system matters more than looking at the unit.
Losing pressure means water is escaping somewhere — a relief valve discharging, a failed expansion tank, weeping circulator flanges, or cracked boiler sections. It is worth addressing quickly: every gallon of fresh make-up water brings dissolved oxygen with it, and oxygen is what corrodes a cast-iron boiler from the inside out.
